Abrazo Advantage Health Plan (HMO) has compiled these guidelines for your reference. The guidelines are developed through research and nationally accepted practices and are based on valid and reliable clinical evidence or a consensus of health care professionals in the particular field.
These guidelines are reviewed and updated periodically as appropriate, and at a minimum of once annually.
These recommendations for care are suggested guides for making clinical decisions. Clinicians and their patients must work together to develop individual treatment plans tailored to the specific needs and circumstances of each patient.
