Abrazo Advantage (HMO) Plan offers many benefits. You get access to leading doctors, specialists and hospitals in your area. Plus, you'll get good prescription drug coverage through Medicare's new prescription drug program, called Part D. Abrazo Advantage (HMO) plan is offered in Maricopa and Pinal Counties.

Abrazo Advantage Plus (HMO) Plan is for people who are on Medicare and qualify for Arizona Health Care Cost Containment System (AHCCCS/Medicaid). It is offered in Maricopa and Pinal Counties. This plan is also available to people on Medicare who will qualify for AHCCCS in the future.

*Please note: Because of the state budget deficit, AHCCCS will no longer pay prescription co-payments for members receiving both Medicaid and Medicare benefits. The pharmacy may require you pay the co-payment amount before you receive drugs covered under Part D Medicare. This is effective as of March 1, 2009.

To enroll into one of our two plans or to learn more about Abrazo Advantage Health Plan, please call us at (602) 824-3900 or 1-888-864-1114. We are available Monday through Friday between the hours of 8:00 A.M. and 8:00 P.M.
TTY/TDD users please call 602-824-3909 or 1-800-489-1472 Monday through Friday 8:00am to 5:00 pm and after hours, on weekends and holidays call 1-800-842-4681 to access the Arizona Relay System.
Enrollment Periods
There are different types of general and special Medicare enrollment periods for Medicare Advantage Plans (Part C) and Medicare Prescription Drug Plans (PDP). Beneficiaries may only enroll in or change plans during an election period that occurs at specific times of the year. Abrazo Advantage Health Plan is a Health Plan that contracts with the Centers for Medicare & Medicaid Services (CMS) and the contract is renewed annually. The availability of coverage beyond the end of the current contract year is not guaranteed. All Medicare Advantage plans are subject to possible contract termination by the Centers for Medicare & Medicaid Services (CMS) and/or reduction in the service area. Should the contract be terminated, current members can select to enroll onto other Medicare plans.
