Case Management Program
Abrazo Advantage Health Plan offers you the opportunity to participate in the Case Management Program when you become overwhelmed by multiple health problems.
We're here to work with you to understand your illness and assist you to become self-sufficient. Not everyone needs to be in the program, but if you have any of the issues below you may want to have your doctor refer you to the program. You may also refer yourself to the program.
Your doctor is responsible for coordinating your care and referring you to specialist when necessary. The health plan care manager works with you, your family or significant other, your doctor and other people who provide care for you to make sure you get the care you need. A care plan will be developed especially for you and you, your family and doctor(s) are part of the plan.

You will receive a phone call from a care manager if you have been identified as being eligible for the program, your doctor has referred you to the program or you have requested to be in the program. You have the right to decide not to participate in the program and may request to re-enroll.
