Abrazo Advantage Health Plan is proud to be a Phoenix-based company that understands local health care options and the unique needs of your community. As a Medicare Advantage plan, we combine medical, hospital and prescription drug coverage when you use our network of doctors, hospitals and pharmacies. We combine the benefits of Medicare Part A, Part B and Part D, and preventive care.

As a member of the Abrazo Advantage Health Plan, you will have access to a 24 hours/7 day a week nurse line to answer your medical questions. We also offer bilingual providers and translation services to assist members with their medical care. Many of our health care providers speak languages other than English. If you need help in your language, or if you have a hearing impairment, call Member Services for an interpreter.

Enrollment

Joining the Abrazo Advantage Health Plan is simple. Just set up a meeting with one of our Health Care Plan Associates. They will go over our plans with you and answer any questions you may have.
