WHEN CAN YOU ENROLL?
- If you have both Medicare and full Medicaid coverage, you can join a plan at any time.
- If you're new to Medicare, you can join during the period that starts three months before the month you get Medicare, and ends three months after you get Medicare.
- If you're eligible for a Medicare Advantage Plan you can join or switch Medicare Advantage Plans during the Annual Election Period (AEP) from October 15 through December 7.
- In special circumstances, Medicare may give you an opportunity to switch to another plan. For example, if you permanently move out of your plan's service area; if you qualify for extra help paying for prescription drugs; if the plan stops offering prescription drug coverage; or if you enter, live in, or leave a nursing home.

Abrazo Advantage Plus (HMO SNP)
Serving Residents in Arizona's Maricopa, Mohave, Pima, Pinal & Yavapai counties
If you're Medicare and Medicaid (AHCCCS) eligible and want medical, hospital and prescription drug coverage, plus no cost extras like dental, hearing and vision coverage, over the counter benefits, chiropractic and podiatry.

Medicare beneficiaries may enroll in Abrazo Advantage Health Plan (HMO) through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov.
Extra Help for Prescription Drugs
You may be able to get Extra Help to pay for your prescription drug premiums and costs. To see if you qualify for Extra Help, call: 1-800-MEDICARE (1800-633-4227). TTY users should call 1-877-486-2048, 24 hours a day/ 7 days a week; the Social Security Office at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY users should call, 1-800-325-0778; or Your Medicaid Office.
